Tethered oral tissue refers to a condition in which the frenulum, the small band of tissue connecting either the tongue or the lip to the mouth, is too short, too thick, or attached in a position that restricts natural movement. When this restriction affects the tongue, it is called ankyloglossia, more commonly known as tongue tie. When it affects the upper lip, it is known as a lip tie.
According to the pediatric medical resource PediatricEducation.org, the incidence of tongue-tie ranges from 2.8% to 10.7% depending on the study, with some estimates even higher in newborns. These conditions are present from birth and can significantly affect a child’s daily life if left untreated. Identifying them early gives children the best opportunity for healthy development.
The effects of tongue-tie and lip-tie vary widely depending on the child’s age and the severity of the restriction.
The most immediate concern for newborns with a tongue tie or lip tie is difficulty breastfeeding. A restricted tongue affects the infant’s ability to latch correctly, which can lead to poor milk transfer, inadequate weight gain, and significant discomfort for nursing mothers. A lip tie can also prevent the baby from forming a proper seal during feeding. Older children with untreated tethered oral tissue may have trouble chewing, swallowing certain foods, or maintaining oral hygiene. In some cases, a tight lip tie contributes to a gap between the upper front teeth or makes thorough brushing more difficult. A frenectomy is the procedure used to release the restrictive tissue.
